At the Atlas General Hospital within the Center for General Surgery, a team of doctors performs a large number of laparoscopic gallbladder surgery (cholecystectomy). Cholecystectomy is a minimally invasive surgical procedure that removes a diseased gallbladder. This technique has essentially replaced the open technique for routine cholecystectomies since the early 1990s.
Laparoscopic gallbladder surgery
Gallbladder removal – cholecystectomy is performed instead of the classic surgical incision of about 15 – 20 cm through 3 of 4 incisions with a diameter of 0.5 to 1 cm, without cutting the structures of the anterior abdominal wall. Trocars are placed in certain places on the abdomen, through which a camera is placed – a telescope and working instruments.
The surgery is performed so that the patient is in the appropriate position under general anesthesia. A thin needle (Veress) is inserted into the patient through the navel, through which gas (usually CO2) is insufflated. The gas in the abdomen has the task of lifting the anterior abdominal wall and thus creating a “working space”, necessary for further surgical work.
After reaching the working pressure, a trocar is placed through which a telescope is introduced – a laparoscope which is connected to a camera and a monitor that displays a picture of the internal organs. Then, the remaining trocars are placed, through which working instruments are placed, with the help of which the surgeon releases the biliovascular elements (artery and gallbladder line), then they are ligated – clipped and cut.
After that, the gallbladder is removed from its lodge and pulled out of the abdomen through one of the ports.
Sometimes it is necessary to place an abdominal drain through one of the ports, and then perform gas desulfulation, extraction of ports and sewing incisions. After that, the patient is taken out of anesthesia and the surgical intervention is completed.
Recovery after gallbladder surgery
After gallbladder surgery, the patient can get up a few hours later and start taking fluids and light foods, and leave the hospital the same day or the next day. The patient’s activities depend on how he feels, and early activation and walking are usually recommended. A check-up is scheduled for seven days.
In a small number of patients (2 – 4%), this operation cannot be completed due to the impossibility of visualizing the biliovascular elements and structures and the impossibility of manipulating them. In this case, the so-called conversion is done – the transition to an open – classic operation. This is not a complication, but a matter of surgical assessment to safely complete the surgical intervention.

After laparoscopic gallbladder surgery, the patient returns to work after 7 days, unlike classic surgery, where that period is 4 to 6 weeks.
